About the role

Reporting to the Director of Community Outreach, the Registered Nurse will assist with the responsibilities of the Pelee Island Clinic. The Registered Nurse is deployed out to the remote community of Pelee Island to provide interdisciplinary Primary and Urgent Care, and social supports. They will work at the Pelee Island Clinic, partnering agencies with the client population, nursing team, and partners. They will also facilitate virtual support to their patients by communicating with the ESHC Mobile Medical Support (MMS) team and Virtual Emergency Department (ED) to manage urgent and emergent assessments, and stabilizations of acute emergency cases for transfer to mainland higher level of care. The successful applicant will be required to travel to Pelee Island to work regular clinic hours, as well as respond to calls that occur after-hours on an on-call basis.

JOB FUNCTIONS
Communication

  • Facilitate and implement systems which promote and maintain effective communication and collaboration with physicians and nurses in the community, the MMS frontline team, and community service providers
  • Collaborate with physicians, nurse practitioners and staff of ESHC Virtual ED
  • Collaborate in the development of Medical Directives for the delivery of remote patient care on Pelee Island
  • Consult with Physicians according to the CNO’s document, Standards for Practice – RN (EC), 1988
  • Collaborate with community partners and patient advocacy groups to identify needs and to develop primary care programs that are culturally appropriate

Staff Education

  • Maintain and enhance a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
  • Coach and teach staff in the health care management of selected patient populations, both formally and informally
  • Assist staff as appropriate in the development of clinical decision making and problem-solving skills through case consultation, clinical demonstration, case reviews, etc.
  • Facilitate own professional development and maintain clinical competency through formal educational programs and mentoring opportunities with consultant(s) and peers
  • May precept postgraduate students from various programs

Assessment and Reporting

  • Maintain appropriate records and documentation of clinical and educational activities
  • Participate in the program evaluation process and exhibit strong personal commitment to continuous quality improvement
  • Participate in the development of programs and services geared towards culturally appropriate and safe care
  • Conduct needs assessments
  • Develop and participate in quality-monitoring programs for the services and routes results to service managers

Leadership

  • Provide leadership in the development and revision of scientifically based clinical standards for a selected patient population
  • Demonstrate leadership within clinical team and with community partners to support strategic direction and planning
  • Monitor compliance with clinical standards
  • Demonstrate leadership skills, expertise, and an in-depth knowledge base consistent with advanced nursing practice
  • Identify need for change to clinical, education and research issues in nursing practice

Other

  • Practice according to the competency areas defined by the Colleges of Nurses of Ontario
  • Health Assessment – performs a comprehensive health/symptom directed assessment
  • Health Promotion and Disease Prevention – implements strategies to promote health and prevent disease with individuals, families and communities
  • Family Health – synthesizes information from individual clients to identify broader implications for health within the family
  • Community Development and Planning – utilizes community assessment data to identify community needs and resources and develops, implements, and evaluates appropriate programming in collaboration with other providers
  • Program planning and development skills in collaboration with Community Partners

QUALIFICATIONS

  • Current certification of competence as an RN in Ontario
  • Minimum 2 years of remote nursing experience required
  • Experience in Emergency, Critical Care, Primary Care, Triage, and Community an asset
  • ACLS
  • PALS
  • CTAS
  • NPR
  • 12-Lead ECG
  • Maintains and enhances a current level of knowledge relative to professional practice as well as continuing education requirements
