Leduc LINX Connect Centre is an agency that provides services to individuals with developmental disabilities.
LINX is seeking a compassionate and motivated Outreach Worker to join our team in supporting individuals with developmental disabilities to achieve greater independence and personal success in their daily lives. Reporting to the Program Operations Manager (POM), the Outreach Worker plays a vital role in advocating for clients, supporting their personal goals, and fostering inclusive community connections.
Key Responsibilities:
- Advocate with and for clients to access appropriate services and community resources.
- Assess client needs and collaboratively develop personal goals and action plans.
- Provide support to enhance life skills, social skills, and independence.
- Assist clients in accessing health care, housing, financial, and community-based resources.
- Foster opportunities for social connection and personal development.
- Support clients in building routines, managing daily living tasks, and developing coping and problem-solving skills.
- Provide crisis intervention support using agency-approved approaches (e.g., Mandt).
- Facilitate practical changes in clients’ environments to promote well-being and self-esteem.
- Attend team meetings, client planning sessions (ISP meetings), and debriefings as required.
- Support the development and implementation of client Individual Support Plans (ISPs).
- Maintain clear, professional, and timely documentation including daily notes, incident reports, and behavioral tracking.
- Serve as a positive representative of LINX within the community.
- Transport clients within Leduc (and occasionally to Edmonton) for appointments, errands, and activities.
- Coach clients in everyday life skills such as budgeting, meal preparation, apartment management, and routine development.
- Perform other related duties as assigned.
Qualifications:
- Experience in community or social services, disability services, or a related human services field preferred.
- Strong interpersonal and communication skills with the ability to build trust and rapport.
- Demonstrated ability to advocate, problem-solve, and provide effective support in various situations.
- Familiarity with person-centered planning, positive behavior supports, and community inclusion principles.
- Crisis intervention training (e.g., Mandt) is an asset or willingness to obtain.
- Valid driver’s license and access to a reliable vehicle required.

This is a 30-hour per week position, scheduled as 6-hour shifts, 5 days per week. Shifts will typically fall within one of the following timeframes:
- 9:00 AM – 4:00 PM (with a 1-hour unpaid lunch from 12:00–1:00 PM)
- 1:00 PM – 7:30 PM (with a 30-minute unpaid meal break from 4:00–4:30 PM)
While a consistent number of hours is guaranteed, the specific schedule will vary based on program and client needs. Some weekend shifts may be required.
