Description
Remote Care Management (RCM) refers to a model of care in which clinical teams and health care providers remotely support a patient’s care plan via virtual tools. The Remote Care Coordinator will be responsible for helping the organization execute on this program in partnership with Ontario Health, the Cornwall and Area Ontario Health Team and the Cornwall SDG Paramedics.
RCM fait référence à un modèle de soins dans lequel les équipes cliniques et les prestataires de soins de santé soutiennent à distance le plan de soins d’un patient via des outils virtuels. Le RCC sera chargé d'aider l'organisation à exécuter ce programme en partenariat avec Santé Ontario, l'équipe Santé Ontario de Cornwall et de la région et les ambulanciers paramédicaux de Cornwall SDG.
Qualifications
- Current registration with a professional college
- Three years past experience with geriatric populations in either long term care or hospital environments preferred
- Excellent critical thinking, problem solving skills and ability to proactively achieve resolutions both through direction and negotiation and make decisions rapidly.
- Past Experience with Discharge Planning or Care Coordination
- Bilingualism (French) at an advanced level is required

St. Joseph's Continuing Care Centre (SJCCC) is home to Hotel Dieu Hospital and St. Joseph's Villa. Located in downtown Cornwall, Ontario, SJCCC offers Rehabilitation and Long-Term Care services to adults at its 208-bed state-of-the-art healthcare facility.
