Job Title: Linkage to Care Coordinator
Job Overview:
The Linkage to Care Coordinator plays a vital role in supporting client care by coordinating internal referrals to Elevate NWO’s health and wellness programs. Positioned at the reception area, they serve as the first point of contact—often the first face and voice someone encounters when reaching out to the organization. This role combines patient advocacy, harm reduction support, and administrative coordination to improve access to healthcare and social services for individuals living with or at risk of HIV, Hepatitis C, and related health concerns.
Key responsibilities include managing incoming calls, preparing and tracking referrals, maintaining accurate data entry, and ensuring interdepartmental communication runs smoothly. They also help maintain a clean and welcoming reception space, while supporting clients in navigating services and connecting them with the appropriate programs within the agency.
Qualifications & Skills
- Post-secondary education in Social Services, Health Sciences, or a related field.
- Experience working with diverse and marginalized populations, with a strong understanding of harm reduction principles.
- Proficiency in electronic medical systems and administrative coordination.
- Effective communication, problem-solving, and decision-making skills.
- Ability to work in a fast-paced environment and reassess priorities as needed.
- Strong organizational and multitasking abilities.
- Ability to facilitate group sessions and community events.
- Must be comfortable in an environment where discussions about sex, sexual health, substance use, and harm reduction are common.
- Valid Ontario driver’s license and clean driver’s abstract; access to an insured vehicle is an asset.
- Must pass a Vulnerable Persons Criminal Reference Check.
