Position Summary:

Manager Patient Flow & Transitions is accountable for ensuring timely and appropriate access to Inpatient bedded services, efficient flow and robust discharge practices where patient and family goals are supported and legislation is upheld. This Manager will liaise with internal and external customers to ensure utilization of beds are appropriate, that HDGH is meeting the needs of our community for Inpatient services and that all flow activities are patient and family centered. Manager Patient Flow & Transitions is responsible for providing transparent and timely reporting and communication of ALC and bed utilization and flow data, both internally at HDGH and externally to Ontario Health, ATC – Cancer Care Ontario and with Regional partners. HDGH Intake department and Social Work department will directly report to this manager.

Qualifications:

RESPONSIBILITIES:

  • Directly manage, coach and mentor all Intake and Social Work staff within the Rehab, Complex Care and Palliative care programs, and act as a resource for the Social Workers in the mental health program regarding ALC and Patient Flow strategies.

  • Attend and provide data for weekly regional hospital operations meeting, reporting on patient flow and ALC indicators.

  • Analyze patient flow data daily (capacity management, HDGH waitlist), mitigating barriers to intake and discharge working with teams at HDGH, Acute care hospital and with community partners. Communicate HDGH utilization status with acute care weekly- supporting transparent communication and provide opportunity to identify barriers to flow.

  • Act as ALC Coordinator for HDGH ensuring education for front line users, reporting, and data collection are kept current with Provincial Standards. Report on ALC trending and work with outside agencies to mitigate ALC days. Develop and present ALC education to front line staff including physicians, and provide yearly refreshers courses for all Inpatient programs.

  • Develop and present educational materials regarding HDGH and inpatient programs and resources, patient flow initiatives, Bill 7 and other policies and procedures affecting patient flow and transitions.

  • Act as resource in the areas of patient access and flow, ALC management and discharge planning both internally (Executive Team, Leadership) and with outside agencies (Ontario Health at Home, ALSO, BSO, LTC). Participate in monthly flow meetings with Ontario Health, Acute care partners and Ontario Health at home.

  • Attend and represent HDGH at provincial and regional tables related to patient transitions, Rehab Care Alliance Framework and ALC.

  • Analyze monthly budget reports and prepare monthly variance reports. Prepare benchmarking research with peer hospitals to support best practice and budget evaluation to find efficiency.

  • Support Medical Director Restorative Care with patient flow initiatives involving physician education and workflow development.

  • Assist in the development and implementation of the organization’s strategic plan and measure progress and success by setting and monitoring specific KPIs.

  • Develop and lead patient transition strategies to support Ontario Health Operational Directives, Ontario Health Best Practice Transition Guidelines, QIP, Corporate Strategic Plan, ALC Management Framework, Regional OHT priorities, and Rehab Care Alliance Framework. Support this work by engaging the Physician group, Leadership, Acute Care Regional Partners, front line staff, Ontario Health at Home and Community partners as appropriate.

  • Monitor and analyze corporate and departmental flow scorecards and indicators. Develop and monitor QIP targets as related to patient flow, Intake and Discharge and commit to quality improvement in those areas.

  • Develop Corporate and Departmental Policies and Procedures related to Intake, Access and Flow, Bed Utilization Management, Repatriation, Internal Surge, Social Work, ALC Management, Admission and Discharge Planning.

  • Develop positive labor relations with union and non-union staff respectful of the contract agreements. Develop respectful, transparent relations with Community partners to ensure quality and timely care for our patients and families and respectful partnership among all care providers.

  • Ensure appropriate electronic systems and interfaces are used to maximize data quality, efficiency, safety in daily work and reporting.

  • Provide leadership and support daily, removing barriers for Intake and Social Work departments to provide efficient services. Lead yearly departmental staff meetings with Intake staff and physicians and lead monthly Social Work meetings to advise of budget, financial position, corporate strategies and initiatives

  • Perform monthly quality chart audits for Social Work and Intake documentation to ensure HDGH and college standards are met.

  • Project Lead for implementation of patient flow strategy projects, (e.g electronic patient flow dashboard, Oracle ALC form), ALC avoidance framework strategies, Estimated Data Discharge, electronic waitlist. Develop patient flow tools/ education programs/ training manuals for implementation of these flow initiatives, upgrades to ALC reporting systems relating to patient flow.

  • Provide support and authority as per escalation algorithm in Discharge and Admissions policies. Lead interdisciplinary complex admission and discharge meetings as required.

  • Meet weekly with Manager Ontario Health at home as best practice in ALC mitigation.

  • Recommend strategies to Executive Team to mitigate barriers to flow including strategies to maintain occupancy in consideration of funding formulas, and recommend practices for continuous process improvement. Provide Executive Team/ Leadership patient flow data weekly/monthly, analyzing access to care and barriers to discharge.

  • Reconcile daily WTIS and ALC data, and monitor submissions for any errors. Respond to ATC to meet compliance deadlines. Accountable for monthly ALC data submissions and reconciliation with ATC.

  • Responsible for payroll and scheduling of Intake and Social Work staff. Responsible for hiring, disciplinary actions, performance evaluations, attendance/sick time management.

QUALIFICATIONS:

  • Ability to work independently and as part of a interprofessional team.

  • Advances skills in competencies of critical thinking and problem solving.

  • Demonstrated excellence in interpersonal and communication skills – both written and verbal

  • Proven management experience, including development, goal setting, and performance management

  • Project management training supporting ability to lead projects, assignments and working effectively with all partners

  • Excellent conflict management, written/verbal communication, resolution and negotiation skills

  • Experience and proven understanding of ALC definitions, data reporting to MOH

  • Experience with hospital software applications (Oracle, Power Chart, Capacity Management)

  • Knowledge of principles, practices, laws and regulations related to the provision of health care within the community and hospital setting

  • Proven computer skills in Microsoft Office, basic report writing, excel

Education and/or Certification:

  • Undergraduate Degree in Health-Related Field - Required

  • Master’s Degree in Health-Related Field – Preferred

  • Registered with relevant governing profession college – Required

  • 5-7 years in related patient Management/ Patient Flow Hospital experience - Required
