The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church & Bloor St), Harbour Light and a Specialized Care Centre (Victoria Park & McNicoll).

12 HOUR SHIFTS (Days/Nights/Weekends)

Position Summary:
The Personal Support Worker (PSW) is a member of the inter-professional team who works in collaboration with team members in the provision of patient care following the principles of Patient Centered Care and the Mission, Vision and Values as well as Policies, Procedures and Standards of Care of The Salvation Army Toronto Grace Health Centre (TGHC). We are looking for a highly motivated individual and works collaboratively with the interdisciplinary team.
Main Responsibilities:
- Assists patients with Activities on the Daily living (i.e., assistance with meals, patients’ transfers and repositioning, including the use of mechanical lifts as per policy as well as oral hygiene & toileting)
- Performs activities or tasks delegated by a Registered Nurse (RN) or Registered Practical Nurse (RPN) in compliance with Regulated Health Professions Act. RNs or RPNs will be responsible for the ongoing supervision of the PSW in the performance of such tasks
- Ensures a safe environment for clients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavioural, physical or cognitive changes
- Reports safety concerns ( including environmental hazards) immediately to the Patient Care Manager, Registered Nurse, Registered Practical Nurse, Occupational Health Nurse or administrator on call, as applicable
- At mealtimes ensure that: food tray tables are cleared and clean to receive trays, patients are properly set up for their mealtime, assist patients with the consumption of their meal as needed and load food trays onto the racks after mealtime is over
- Make beds and assist Environmental Services staff with patient personal laundry
- Responsible for isolation bag removal and dirty linen bag removal from patient rooms
- Clean and de-clutter patient refrigerators
- Documents and maintains related patients’ records and progress notes as per TGHC policies.
- Attends Transfer of Accountability meetings upon arrival to the assigned shift
- Attends daily Patient Safety Huddles and education sessions as applicable
- Perform other duties as required as assigned by the Patient Care Manager
Qualifications and Education:
- Successful completion of a PSW program from an Accredited Institution ( i.e. by the Ontario Ministry of Training, Colleges, and Universities)
- Current Basic Cardiac Life Support (BCLS) Certificate
- Behavioral management training (i.e., PIECES. U-first- CPI) is considered an asset
- Experience working with persons with Dementia or cognitive impairment
- Flexible, adaptable to changing work environment
- Able to establish therapeutic relationships with patients and families
- Commitment to continuous improvement, learning and evidence-based practice
- Demonstrated resilience to adapt in an ever-changing environment
- Demonstrated problem solving and critical thinking skills
- Highly motivated, compassionate individual with excellent interpersonal and communication skills
- Excellent written and verbal skills in English
- Strong time management skills, excellent attendance record
- Demonstrated skills in applicable computer software applications
- Ability to meet bone fide occupational requirements as outlined in the Job Demands Analysis
- Must have the physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position
- Must have received at least two doses of a COVID-19 vaccine approved by Health Canada prior to start date, and submit evidence of such. Must maintain Covid-19 Vaccination schedules as approved by Health Canada Medical exemptions or any other kind of requested exemption based upon the Hospital’s obligations pursuant to the Ontario Human Rights Code will be considered on a case-by-case basis
