Rexdale Community Health Centre is a non-profit, multi-service centre that provides primary health care, settlement, and community development activities for women, families, youth, adults, and seniors of diverse cultural backgrounds. The Centre is committed to providing accessible and sustainable programs and services to diverse communities. The Centre has service delivery that incorporates principles of advanced access, utilizing group visits and self-management models. The Centre is currently seeking a part-time Nurse Practitioner for a contract who will work with the mental health team to provide mental health support and primary health care to black children, youth, and mothers. The successful candidate will be a member of the Centre’s Clinical team.
Job Summary:
As part of Ontario’s pediatric Recovery Initiative, the Part-Time Nurse Practitioner (NP) will provide culturally responsive, community-based mental health care to Black children, youth, and families. This role supports a range of innovative programs, including early psychosis intervention, mobile mental health clinics, and prenatal/postnatal care for Black mothers and children. The NP will work collaboratively with interdisciplinary teams and community partners to improve access to mental health services and promote wellness in our community.
Responsibilities:
- Early identification of mental health signs in the 12-29 years, facilitating prompt action to prevent condition escalation.
- Conduct comprehensive assessments and diagnoses, creating personalized care plans for ongoing mental health support.
- Order/interpret results of screening, diagnostic lab tests, x-rays and ultrasounds
- Educate families and advocate for policies that support mental health within healthcare systems.
- Integrate mental and physical health care to provide comprehensive child care.
- Promote a team-based approach to pediatric health, improving outcomes through coordinated efforts.
- Provide mobile mental health and addictions services to youth in the communities.
- Commitment to evidence-based methods is critical, with NPs employing the latest therapeutic and pharmacological interventions.
- Practice from a holistic health approach; NP assesses the full spectrum of factors affecting a child's well-being, from clinical symptoms to environmental and psychosocial aspects.
- NP will lead by example, enhancing care standards and fostering best practices within healthcare teams.
- Collaborate with other programs such as the Etobicoke Pre- and Post-natal Program.
- Assess the need for and execute health promotion, as well as primary and secondary prevention strategies for individuals, families, and communities.
- Explore innovative service delivery approaches.
- Advanced knowledge and proficiency in current, evidence-based methods and practices of primary care delivery with an emphasis on mental health, health promotion, risk reduction and injury prevention
- Maintain and develop competence through appropriate continuing education methods
Qualifications and Assets:
- Baccalaureate nursing degree with Extended Class Certificate of Competence from CNO in primary health care
- Registration with the College of Nurses of Ontario with professional liability insurance
- Affiliation with the Nurse Practitioners’ Association of Ontario (NPAO)
- Experience working as a Registered Nurse Practitioner in a community setting is an asset
- Knowledge of the Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
- Familiarity with electronic medical records, e.g. NOD, PSS, is an asset
- Proficient with computer software including MS Office, Internet and email
- Ability to work effectively both independently and as a member of an interdisciplinary team
- Demonstrated ability to work well with women, youth, marginalized population and diverse ethno-racial and cultural groups
- Demonstrated experience in all aspects of program development and implementation
- Awareness of and sensitivity to the health issues of a diverse community
- Sound knowledge of health promotion and disease prevention is an asset
- Excellent verbal and written communication skills
- Demonstrated ability to multitask, be flexible and organized
- Must be able to provide criminal record check clearance
- Ability and willingness to travel
- Knowledge of the Rexdale community
- Ability to speak a second language (e.g. French, Spanish, Punjabi, and Somali) will be an asset.
Job Types: Part-time, Contract
Pay: $56.81-$68.44 per hour
Expected hours: 17.5 per week
