Location : Unity Health Toronto – St Joseph’s Health Centre
Position Type: Full Time Temporary (18 Month Maternity Leave)
Position Summary
- Provision of direct, patient-focused care in a multi-disciplinary team environment.
- Provision of drug information and development and/or review of medication protocols specific to clinical practice.
- Support of the medication distribution system.
- Preceptorship of pharmacy students for on-site clinical education.
- Projects and safety initiatives
- Provision of education, presentations and in-services for the department of pharmacy, and for the inter-disciplinary Outpatient Oncology clinic.
- Provision of education, presentations and in-services for the department of pharmacy and medicine program.
Qualifications
- Graduation from recognized pharmacy degree program.
- Licensure with the Ontario College of Pharmacists.
- Hospital Pharmacy residency an asset.
- Previous hospital experience in direct patient care in a team environment an asset.
- Self-motivated team player, reliable, cooperative, and patient-focused.
- Excellent interpersonal and communication skills.
- Satisfactory attendance record.
