Location : Unity Health Toronto – St Joseph’s Health Centre

Position Type: Full Time Temporary (18 Month Maternity Leave)

Position Summary

  • Provision of direct, patient-focused care in a multi-disciplinary team environment.
  • Provision of drug information and development and/or review of medication protocols specific to clinical practice.
  • Support of the medication distribution system.
  • Preceptorship of pharmacy students for on-site clinical education.
  • Projects and safety initiatives
  • Provision of education, presentations and in-services for the department of pharmacy, and for the inter-disciplinary Outpatient Oncology clinic.
  • Provision of education, presentations and in-services for the department of pharmacy and medicine program.

Qualifications

  • Graduation from recognized pharmacy degree program.
  • Licensure with the Ontario College of Pharmacists.
  • Hospital Pharmacy residency an asset.
  • Previous hospital experience in direct patient care in a team environment an asset.
  • Self-motivated team player, reliable, cooperative, and patient-focused.
  • Excellent interpersonal and communication skills.
  • Satisfactory attendance record.
