About the role

Position Title: Nurse Practitioner

Reports To: Manager, Allied, Access and Mental Health

Position Summary:

The Nurse Practitioner, Primary Care (NP) will work within a multidisciplinary care team with the goal of supporting optimal health in our rural communities. The NP will work with a high degree of autonomy in a flexible, responsive manner to provide comprehensive services across a variety of settings, including CRPCN operated clinics. The NP will work to full scope of practice to help meet the following priorities in alignment with CRPCN objectives:

  • Increase access to primary health care
  • Meeting unmet demand for primary health care services
  • Provide care to underserved populations; and
  • Support chronic disease management

Key Responsibilities:

· Primary healthcare service delivery including but not limited to screening, chronic disease management, prenatal care, and mental health counselling

· Work within a multidisciplinary team to improve access to primary care services for unattached patients

· Case management of patients with highly complex needs

· Apply holistic approach to individual patient needs and develop health care plans in collaboration with multidisciplinary team to support patient in their health journey

· Know and follow current best practice guidelines to ensure evidence-based practice

· Actively lead/engage in patient navigation and clinical process redesign

· Lifestyle counseling for patients with, or at risk for, a wide range of chronic and/or complex health conditions

· Participate in CRPCN and ongoing quality improvement initiatives

· Participate in CRPCN evaluations as required

· Work in partnership with allied health and collaborative community partners to increase access

· Other duties as required within Nurse Practitioner scope of practice

Educational Requirements:

· Master’s degree in nursing, with Nurse Practitioner certification

· Fully licensed and in good standing with regulatory body

· Current BCLS certification for health care providers

· Mental Health and Addictions experience an asset

Experience and Skills Required:

· 5 or more years of nursing experience, preferably in a rural primary care setting

· Experience working within a multidisciplinary care team model

· Experience working with the Indigenous population

· Experience working with Addictions & Mental Health programming & principles

· Knowledge of chronic disease management principles and processes using current best practice guidelines

· Knowledge of EMRs and clinical process redesign is an asset

· Knowledge of and interest in principles of a population-needs based approach, primary care, primary health care and the medical home model is an asset

Working Conditions:

· Clinical office environment

· After hours, evening and weekend shifts

· Reporting matrix structure of CRPCN management

· Working under minimal daily supervision

· Travel to additional CRPCN work sites may be required

· Valid driver’s license & access to reliable vehicle required

The Calgary Rural PCN collaborates with communities, family doctors and other health team members to do whatever we can to help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through support we have in our member clinics, such as nurses, dietitians, and other health care professionals. We collaborate to provide tools and education to empower patients to work on their everyday health needs.
