Your Opportunity: The Outreach Worker will engage in case management, system navigation, and provide direct support by connecting with patients in the community and in their homes. You will work closely with the Social Worker to develop, implement, and support the execution of personalized care plans, accompany patients to appointments, and ensure that they receive the care they need. The role requires initiative, a deep commitment to patient care, and a passion for improving outcomes for individuals facing complex health and social challenges. As the role and team evolve, you will help shape the direction and impact of our outreach efforts.

If so, we are looking for a future colleague who has… A Diploma or Degree in Social Work or other health or health equity related discipline and/or significant experience in a related field such as Peer Support, Community Health, Gerontology, Health Equity, Disability or Addictions. Registered and in good standing with a professional college preferred, but not required (e.g. OCSWSSW, CNO, COTO) Experience in health and/or community/social service settings. Experience working with diverse partners in Hamilton or other communities. Expertise or additional training in addressing social determinants of health, health equity, addictions, gerontology and mental health. Experience and/or training in de-escalation and/or non-violent crisis intervention. Experience as a consumer or caregiver to an individual with chronic and complex health issues.

What is a Family Health Team?
In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients.

HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. We serve over half of the population of Hamilton.
