Position Overview
The Navigator is the Intake Worker responsible for coordinating and managing the referral and intake process for children and youth referred to the rehabilitation program. The role ensures timely, equitable, and clinically appropriate access to services (e.g., OT, PT, SLP), supports families through the intake process, and facilitates effective communication between families, referral sources, and the interprofessional team.
The Intake Worker functions as the first point of contact for families and plays a key role in service navigation, triage, service coordination and waitlist management.

What we are looking for
- Bachelor degree in social work.
- Member in good standing with a professional college in Ontario.
- Experience in coordination, navigation
- Good interpersonal skills, empathetic, able to quickly develop therapeutic relationships, capable of managing difficult clients and obtain the confidence of clients.
- Comfortable with telephone interventions.
- Excellent skills in assessment, analysis, rapid decision-making abilities based on a solid clinical judgement.
- Good knowledge of community resources and of the navigator role.
- Demonstrates good initiative, researches missing data and clearly transmits ideas and information.
- Capable of managing multiple requests, flexible and easily adaptable to quick changes.
- Able to work in a multidisciplinary team and maintain harmonious relationships with colleagues.
