Founded at the turn of the millennium, Peterborough Family Health Team (PFHT) supports the primary health care sector in Peterborough City and County. As one of the largest Family Health Teams in Ontario, the organization works with over 100 primary care physicians providing comprehensive care to their patients. Collectively, we have over 100 inter-professional healthcare providers, comprised of: mental health clinicians, nurse practitioners, pharmacists, a physician assistant, registered dietitians, registered nurses and registered practical nurses. In addition to one-on-one care, PFHT administers many programs, services, and workshops as a means to redirect patients from the providers practice to a more focused area of care. This approach increases capacity at the practice level while providing patients with an opportunity to learn how to manage their health. These programs, workshops, and services are offered at no charge and derive from irrefutable patterns displayed in the perpetual symptoms patients present. PFHT is a Queen’s residency site, allowing our primary care physicians to have the chance to teach new physicians. As Peterborough has a robust medical community, the PGY1s and PGY2s have the opportunity to not only work in primary care but also in emergency care, palliative care, sports medicine, subacute care and so much more.

Provide family physician services as part of the Chemong Family Health Organization, working within a collaborative model of care. Physicians operate their own clinics and manage their clinic costs and overhead while coordinating with multidisciplinary colleagues and participating in organized extended-hours coverage.

Provide front-line remote and on-site IT support to PFHT staff, physicians, and partner family health organizations, including troubleshooting devices, software, networks, accounts, and clinical systems. Assist with equipment deployment, infrastructure and digital health support, cybersecurity and privacy practices, and escalate complex issues to the Information Technology Specialist.

Lead and manage physician-led interprofessional teams and assigned programs, including staff supervision, coaching, performance management, operational coordination, and stakeholder collaboration. Ensure regulatory compliance, effective use of resources, budget oversight, quality improvement, outcome measurement, and alignment with organizational and provincial primary care priorities.

Promote PFHT programs and services through coordinated communications, community outreach, events, and partnerships, while identifying opportunities to improve awareness and participation. Develop sustainable promotional processes, tools, and documentation, and support consistent messaging across PFHT channels and initiatives.

Provide comprehensive primary care, including health assessments, screening, diagnosis and treatment of illnesses, chronic disease monitoring, prescribing, and health promotion. Collaborate with physicians and an interdisciplinary team, consult with physicians as required, document statistical information, and attend team meetings.

Processes accounts payable and receivable transactions, vendor payments, customer invoices, payroll-related payments, and journal entries while maintaining accurate accounting records. Completes reconciliations and supports month-end and year-end closing, payroll, financial reporting, and process improvement activities.
