The Role: Case Manager Youth Diversion is a short-term housing support program for youth ages 16-24. The case manager supports young people on a one-to-one, short-term basis, building upon existing strengths, removing barriers, and supporting them with their goals to build financial and/or housing stability. Key Tasks & Responsibilities * Provide one-to-one short-term support (1-6 months) to youth. * Assist youth in building upon their existing strengths, removing barriers, and supporting them with their goals to build financial and/or housing stability. * Assist youth to build on existing and explore new natural supports. * Connect youth to community resources to facilitate the development of supportive relationships and connection to their communities. * Support youth to navigate the process of finding and/or stabilizing in housing. * Advocate alongside and on behalf of youth to external actors (e.g., Government services, landlords) to support them to meet their needs. * Facilitate and support collaboration with youth through meetings, case conferences, and more. * Provide financial literacy support and resources to youth. * Work collaboratively and build partnerships with other agencies and services in the community. * Evaluate client progress using accurate case management, evaluation tools and documentation. Knowledge & Skills Expectations * Degree or Diploma in the Human Services Field. * Minimum of 1-3 years’ experience working with individuals and families. * Experience working in an outreach-based program is an asset. * Familiarity with housing first, harm reduction practice frameworks. * Strong cross-cultural communication skills and intercultural competencies. * Knowledge of barriers, and services for new-comer, ethno-cultural, and Indigenous families. * Knowledge of community services and programs. * Knowledge and understanding in the areas of strength-based approach, trauma, attachment, and grief & loss. * Valid driver’s license, driver’s abstract and reliable vehicle with appropriate insurance. * Current Standard First Aid & CPR, ASIST, and TCI training an asset (can be completed after hire). * Able to provide a clear Criminal Record Check and clear Intervention Record Check dated within the last six months. Requirements

Candidates must hold a degree or diploma in the Human Services field and possess 1-3 years of experience working with individuals and families. A valid driver's license and a reliable vehicle are required, along with the ability to pass a criminal record check.
