The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter professional health care team utilizing the nursing process to provide direct nursing care as well as provide case management to individuals across the life span. The LPN is responsible for their nursing practice as defined by the College of LPNS and HCA’s of Alberta (CLHA). In collaboration with the Registered Nurse Case Home Care Manager, the LPN is responsible to assess, plan, implement, case manage and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. This role requires travel within the Chestermere and sometimes the Strathmore area including caring for clients in their homes or in the community. You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Advanced wound care knowledge and experience required. Must be proficient in computer use as role is heavily dependent on computer use. Knowledge, skill, ability and willingness to work to full scope of practice. Ability to use critical thinking skills. Critical inquiry to assess, plan & evaluate. Preferred Qualifications: Experience working with acute clients and/or long-term clients with a focus on chronic disease management. Experience in providing case management services. Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching.

The LPN provides direct nursing care and case management to home care clients across their lifespan within an interprofessional team. Responsibilities include assessing, planning, implementing, and evaluating client needs while traveling to various community locations.

Candidates must have an accredited practical nurse education, a valid CLHA practice permit, and BCLS-HCP certification. Three years of recent home or acute care experience and advanced wound care knowledge are required.
