Your Opportunity: As a Home Care Licensed Practical Nurse, you will report to the Home Care Manager and will be responsible to assess, plan, monitor and evaluate clients in consultation with the Registered Nurse Case Manager. You will be responsible for coordinating and providing quality care to clients in the community and will provide direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. This position is a hybrid clinic and community-based role, providing services within the Home Care Office as well as at community shelters. The successful candidate will work collaboratively with clients, interdisciplinary teams, and community partners to support patient care, coordinate services, and promote positive health outcomes across both office and community settings. Description: As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications: Minimum of two years' experience within the last five years of Home Care experience, although skills and experience relevant to nursing practice in community sector will be considered. Valid Driver’s license and access to a reliable vehicle. An excellent attendance record, reliability and personal suitability must be demonstrated through your employment references. Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal. Preferred Qualifications: Experience in Home Parenteral Therapy (HPT), wounds, lower limb assessments, skin and ostomy care, palliative support and client teaching. Aware of Continuing Care Health Service Standards and the ability to incorporate these standards in your practice.

Requirements
