Position Summary This opportunity is ideal for a Nurse Practitioner seeking to build a thriving primary care practice within a supportive, patient-focused environment. Working alongside an experienced pharmacy team, you will provide comprehensive primary care services, manage both scheduled and walk-in patients, develop long-term patient relationships, and play an important role in improving healthcare access within the community. Key Responsibilities Provide comprehensive primary care services across a broad range of patient needs. Assess, diagnose, treat, and manage acute and chronic medical conditions. Build and manage an ongoing patient panel. Deliver preventative care, health promotion, and chronic disease management. Maintain accurate clinical documentation and patient records. Collaborate with pharmacists and other healthcare professionals to optimize patient outcomes. Contribute to a welcoming, accessible, and patient-centred clinic environment. Qualifications Registered Nurse Practitioner licensed (or eligible for licensure) in Ontario. Experience in primary care, family practice, or community health is preferred. Strong clinical assessment and diagnostic skills. Excellent communication and relationship-building abilities. Passion for delivering patient-centred care.
