Birkwood Village – Fort Madison, IA Starting at $45/hour | Full-time | On-call rotation every 4th weekend Birkwood Village is looking for an experienced MDS / Care Plan Coordinator to join our nursing leadership team. In this role, you’ll lead the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and RAI guidelines — directly shaping the quality of care every resident receives. What You’ll Do Coordinate MDS assessments, ARDs, and timely submissions; stay current on yearly RAI Manual changes Review census changes, progress notes, and Medicare A documentation to catch significant changes in condition Develop and coordinate individualized care plans, revising them with every assessment or change in resident status Represent nursing at weekly Medicare meetings and monthly/quarterly QAPI meetings Champion patient- and family-centered care, including leading Care Plan Conferences Share in on-call coverage, one weekend out of every four Ready to bring your MDS expertise to Birkwood Village?
