Job Summary Reporting to the Client Care Coordinator, the Home Support Worker is part of a health care team that provides assistance to families or persons during times of illness, disability, or inability to properly care for their own needs. The home support worker also supports independence and the ability of the client to remain in their own homes and assists with the activities of daily living. Summary Of Essential Job Functions Provide personal care such as bathing, toileting, and grooming to the client Complete housekeeping tasks, meal preparation, and shopping Provide companionship to the client and accompany them to community events, activities, and appointments Follow written plan of care as approved by the Client Care Manager Provide specialized care such as Alzheimer or palliative care, if required Document and report client progress Promote good mental health for the client Develop a caring relationship with the client and the client’s family Provide emotional and psychological support Ensure a healthy and safe living and working environment while following agency guidelines on lifting and transfers Adhere to and enforces CareGivers’ Code of Conduct Report all Occupational Health and safety hazards and related issues and follows OH&S guideline Minimum Requirements Minimum grade 12 high school education and experience as a home support worker OR Certification as a home support worker (Preferred) OR Willingness to commence in-house training Clear Criminal Record Screening Certificate and Vulnerable Sector Check obtained WITHIN 90 days (Or willing to obtain one BEFORE starting employment) Experience working with seniors and or physically disabled individuals Medical certificate indicating good health Negative TB skin test Up-to-date Immunization Record Current First Aid Certificate or willingness to obtain Abilities Required Willingness to work flexible shifts in various locations Excellent communication and interpersonal skills Willing and able to use IT systems for scheduling and client care purposes Ability to work as part of a team or independently Good decision making and problem solving skills Ability to lift 25 pounds Ability to build good relationships with the client Must be caring, enjoy working with others, have a good work ethic, and possess a positive attitude What's in it for me!?
