Role Description The Eldercare – Care Manager is a full-time, hybrid role based in Pickering, ON, with flexibility for some work-from-home tasks. The Care Manager will oversee client care plans, conduct assessments, and coordinate services to ensure that seniors and individuals with special needs receive appropriate, consistent support. Day-to-day responsibilities include collaborating with caregivers and healthcare professionals, monitoring client progress, and adjusting care plans as needs change. The role involves regular communication with clients and families to provide updates, address concerns, and ensure their preferences are reflected in service delivery. The Care Manager may also participate in training care staff, documenting case notes, and supporting quality improvement initiatives. Qualifications Strong Case Management skills, including care planning, assessment, and coordination of services. Experience in Elder Care, with a focus on person-centered support for seniors and individuals with special needs. Background in Social Services, such as counseling, advocacy, or connecting clients to community resources. Training skills, including the ability to guide and mentor caregiving staff on best practices and safety protocols. Foundational knowledge of Medicine or healthcare concepts relevant to homecare, in collaboration with clinical professionals. Diploma or degree in Nursing, PSW or Health Sciences would be an asset. Excellent communication, documentation, and interpersonal skills, with the ability to build trust with clients and families. Strong organizational skills and attention to detail, with the ability to manage multiple cases and priorities. Knowledge of local community resources and support services in and around the Greater Toronto Area. Ability to work collaboratively in a multidisciplinary team and demonstrate cultural sensitivity and respect for diverse backgrounds.
