The Centre de santé communautaire Hamilton/Niagara is a Community Health Centre established in 1992. Our Centre is a multi-service agency providing health and social services to clients. Our Welland site serves the Niagara region, and the Hamilton site serves Hamilton and Burlington. The Centre de santé communautaire Hamilton/Niagara has adopted a holistic approach to the delivery of its programs; thus French-speaking people of all ages in the Hamilton and Niagara regions have access to a wide array of services. Our programs and services have a particular emphasis on disease prevention and health promotion achieved through the provision of information and education. Services vary slightly between Hamilton and Welland according to the needs and resources in each community. Assess adult clients’ psychosocial needs and provide individual and group counselling or psychotherapy, developing intervention plans with clients. Help design and deliver community mental health programs and workshops, coordinate with internal and external partners, maintain records and statistics, and participate in clinical supervision and team meetings.

Plan and facilitate activities and workshops that support the physical, social, and emotional development of children aged 0–6, encourage parent-child interaction, and provide support to families. Assess children’s needs, coordinate appropriate services, collaborate with community organizations, supervise volunteers and students, and help maintain safe group activity spaces.

Welcome newcomers and assess their needs, develop individual intervention plans, and connect them with appropriate settlement services and community resources. Plan and deliver Welcoming Communities activities, coordinate volunteers and partnerships, support community events, and maintain client records and statistics.

The Nurse Practitioner provides comprehensive primary health care, focusing on health promotion, disease prevention, and the management of common health problems. They work within an interdisciplinary team to coordinate care, manage chronic conditions, and participate in policy development and statistical reporting.

Provide individual and group counseling and intervention services to victims and survivors of human trafficking and gender-based violence. Develop service plans, facilitate therapeutic groups, and collaborate with a multidisciplinary team to improve clients' socioeconomic and health situations.
