Our specialized services range from rehabilitation for post-acute recovery from strokes, surgeries, or accidents, to tailored day programs addressing physical, social, or memory challenges. We prioritize spiritual care and supported living for dementia patients, integrating these vital components into our holistic approach. This comprehensive care model encompasses post-acute rehabilitation programs, complex mental health support, dementia care, supported living at level 2, palliative, and long-term care services. Description: The Licensed Practical Nurse (LPN) works as a member of the multidisciplinary health care team, collaborating in the delivery of holistic care and support to patients/residents and families. The LPN practices to full scope, applying professional nursing knowledge, clinical judgment, critical thinking, and communication skills to support safe care and positive health outcomes. The role is committed to providing person-centred, culturally safe, and inclusive care that respects the diverse backgrounds, experiences, values, and needs of patients/residents and families. The LPN contributes to a culture of learning and professional practice excellence by orienting, coaching, mentoring, and supporting the development of staff, students, and learners. Active registration with the College of Licensed Practical Nurses and Health Care Aides of Alberta (CLHA). Basic Life Support Healthcare Provider (Level C) required. Additional Required Qualifications: Minimum of 1 year of experience in a long-term care or geriatric setting. Basic knowledge of long-term care principles, including chronic disease management, medication administration, and wound care. Strong clinical skills and the ability to perform nursing tasks with accuracy and compassion. Excellent communication skills, with the ability to work effectively with residents, families, and the healthcare team. Ability to demonstrate empathy, patience, and professionalism in all interactions. Physically able to complete all demanding aspects of patient/resident care and transport, e.g. walking, standing, bending, lifting, assuming awkward positions and standing for long durations of time. Must be able to understand written and verbal directions in English. Preferred Qualifications: Certification or additional training in Geriatric Nursing or Long-Term Care Nursing (e.g., Certified Nurse in Long-Term Care (CNLTC)). Certification or training in crisis management techniques, such as Nonviolent Crisis Intervention (NCI) or Therapeutic Crisis Intervention (TCI). Experience with electronic health record (EHR) systems, such as Connect Care. Experience with InterRAI MDS/ LTCF Assessments.

Requirements
