The Community Health Nurse provides independent clinical assessments, care planning, and interventions for clients with complex chronic conditions in home and community settings. They collaborate with interprofessional teams and primary care providers to facilitate transitions and promote health maintenance through direct care and education. Candidates must hold current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of one year of recent clinical experience in acute or community settings treating complex geriatric or adult patients is required.
