Reporting to the Supervisor, Home and Community Care, the Home and Community Care Worker II provides essential personal care and specialized support, including assistance with bathing, grooming, mobility, medication reminders, and individualized care for palliative, rehabilitation, post-acute clients. The Home and Community Care Worker II exercises independent judgement in monitoring health status, identifying risks, and determining when to escalate concerns to health professionals, often in unpredictable and isolated environments where immediate clinical support is not available. These decisions directly affect client safety, well-being, and continuity of care, reducing preventable hospital admissions and alleviating pressure on acute care facilities. The role carries accountability for maintaining strict confidentiality, adhering to health and safety protocols, and advocating for client needs through a cultural lens to ensure client care plans respect Inuit values and traditions. Through these responsibilities, the position plays a vital role in sustaining community health and advancing Government of Nunavut’s commitment to equitable and culturally safe health services. Strong interpersonal and communication skills, problem-solving and judgement in identifying health risks and determining appropriate actions are required for this position. Also, the ability to interpret and convey health-related information accurately, work independently in unpredictable home environments under indirect supervision and physical ability to assist with transfers, mobility, and household tasks are also required. The knowledge, skills, and abilities required for this job are typically acquired through completion of Grade 12 and two years of home care experience, or equivalent life experience providing personal care for family or community members, including Elders or individuals with health challenges. BCLS (Basic Cardiac Life Support), First Aid Certificates, Training in personal care skills or home support and basic computer skills for documentation are assets. This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required. The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in more than one of Nunavut’s official languages would be considered an asset. Knowledge of Inuit communities, culture, land, Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets. Acceptable combinations of education and experience may be considered for this position.
