What is a Family Health Team? In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients. Who is the Hamilton Family Health Team (HFHT)? HFHT is the largest Family Health Team in Ontario. We have over 165 family doctors and over 250 allied health care providers in our organization. Conduct cybersecurity assessments across affiliated healthcare practices, identify risks, and develop prioritized action plans tailored to each practice. Coordinate and monitor security improvements, awareness training, reporting, and stakeholder engagement while helping protect sensitive health information.

Conduct mental health and biopsychosocial assessments and provide evidence-based, patient-centered interventions for individuals, families, and groups across the age span. Coordinate care plans, referrals, case management, outreach, and advocacy with patients and interprofessional teams to address social determinants of health and improve access to services.

Provide administrative and operational support to the Nutrition and Pharmacy Programs, including coordinating meetings, schedules, onboarding, communications, and supplies. Maintain program records and online resources, support clinicians and student learners, and assist with reporting, quality improvement, and special projects.

Triage, prioritize, document, and resolve help desk requests involving hardware, software, systems, user access, and account issues, escalating complex cases when needed. Maintain IT asset records and support onboarding and offboarding, provide on-site assistance at affiliate locations, and offer backup support to the Facilities team.

They are responsible for developing care plans, facilitating community referrals, and assisting patients in navigating complex health and social service systems.
