This is done in collaboration with the family physicians, other health care providers, community organizations and programs and the resident and their family. The Nurse Practitioner is responsible for the assessment, planning, delivery, and evaluation of health care services for long-term care residents. They work in collaboration with family physicians, health care providers, and families to ensure coordinated, patient-centered care.

Candidates must hold a Bachelor of Science in Nursing (BScN) or a nursing diploma, along with a Master of Nurse Practitioner degree. Applicants are required to have at least 3 years of experience as an RN in primary, public, or acute care and must be registered or eligible for registration with the CRNS.
