Humber River Family Health Team (HRFHT) offers a wide variety of specialized services to our patients. Our program and services are delivered in a team based care model that includes physicians and an in-house clinical team of Health Care Professionals (i.e. Nurse Practitioners, Registered Nurses, Registered Dietitians, Social Workers, Chiropodist, Health Promoter/Educator and Pharmacists) alongside other community service providers.

The Nurse Practitioner will conduct comprehensive health assessments and manage a caseload of patients while supporting patient attachment to physicians. They will also contribute to the development and implementation of clinical and community-based health programs.

The Chiropodist will provide comprehensive foot care services, including acute treatment, surgical procedures, and preventive care to patients. They are also responsible for maintaining clinical equipment, documenting patient data, and collaborating with an interdisciplinary healthcare team.
