Job Description Care Information Manager Position Overview The Care Information Manager will oversee the information systems, data workflows, and reporting structures that power the Mino Care Perinatal Pathways Program. This position is essential to maintaining a coordinated, bi-directional data-sharing model that connects hospitals, midwifery clinics, community health centres, and Black-led community organizations. The ideal candidate understands both health data and health systems navigation, including provincial and federal frameworks governing privacy, interoperability, race-based metrics, and health information exchange. Roles/Responsibilities (include, But Are Not Limited To) Information System Management Oversee the development, maintenance, and optimization of care information systems supporting the Perinatal Pathways Program. Ensure systems enable secure bi-directional data sharing with all partner institutions. Support onboarding and training of staff in documentation processes, data access protocols, and system workflows. Data Governance & Reporting Ensure adherence to provincial and federal privacy legislation, PHIPA, and applicable data-sharing standards. Coordinate the collection, disaggregation, and reporting of race-based and programmatic outcome data. Work with partner institutions to align data definitions, reporting cycles, and quality improvement metrics. Systems Navigation & Cross-Institutional Coordination Bridge information needs across hospitals, clinics, social services, and community partners. Support the integration of Mino Care staff into partner systems and platforms. Collaborate closely with the Care Partnerships Manager to ensure operational and data workflows align. Quality Improvement & Analytics Develop data dashboards, summaries, and insights to support monitoring, evaluation, and decision-making. Identify gaps in data capture, documentation, or system performance and propose improvements. Qualifications 3+ years of experience in health information management, health informatics, data systems, or related roles. Strong understanding of provincial and federal health information systems, privacy legislation, and interoperability frameworks. Experience supporting cross-institutional data projects or multi-organization health initiatives. Ability to translate complex data concepts for diverse audiences. Commitment to culturally safe, equity-focused care and data practices. Time Commitment: 20 hours/week Pay: $33,600 annually ($35/hour), Location: Hybrid (Toronto/GTA preferred)
