The Care Coordinator assesses patient eligibility and care needs to develop individualized service plans. They collaborate with patients, families, and healthcare professionals to coordinate access to community resources and monitor care implementation. Candidates must be a registered member of a relevant Ontario regulatory body (Nursing, Physiotherapy, Occupational Therapy, Social Work, or Speech-Language Pathology) with at least one year of community health experience. A valid driver's license, vehicle access, and proof of COVID-19 vaccination are required.
