Planned Lifetime Advocacy Network (PLAN) is a nonprofit organization founded in 1989 to advance the full citizenship, belonging, and leadership of people with disabilities and their families. We are a community of people with disabilities, families, and allies dedicated to creating a world where everyone is valued for their contributions and connected through meaningful relationships. At PLAN, we believe citizenship is about claiming the right to lead a life of purpose, connection, and security within a welcoming and inclusive community. A meaningful life is built on relationships, opportunities to contribute, and the freedom to shape one’s own future. That’s why PLAN focuses on creating personal support networks, fostering inclusive communities, and planning for secure and dignified futures. The Mentor builds trusted relationships with Lifetime Members and their families to develop and sustain personal support networks. They facilitate future-planning conversations and help families navigate systems to ensure community belonging and support during transitions.
