About the Company We are currently seeking an experienced Nurse Practitioner (NP) to join a collaborative healthcare team providing advanced clinical care to frail and elderly patients in the community. This is an excellent opportunity for an NP who is passionate about comprehensive patient care, clinical leadership, care coordination, and working within an interdisciplinary healthcare environment. About the Role This role involves providing advanced clinical care to frail and elderly patients, ensuring comprehensive patient care and effective coordination within a healthcare team. Responsibilities Conduct comprehensive health assessments, including medical histories, physical examinations, and psychosocial assessments. Order and interpret diagnostic tests and procedures within the NP scope of practice. Establish medical diagnoses and develop individualized care plans. Prescribe medications, adjust dosages, and order appropriate treatments and interventions. Act as the Most Responsible Provider (MRP) for assigned patients. Coordinate care with physicians, allied health professionals, community services, patients, and families. Monitor patient outcomes and modify care plans as required. Provide education and support to patients, families, caregivers, and healthcare professionals. Maintain accurate and comprehensive clinical documentation. Participate in interdisciplinary team meetings, quality improvement initiatives, and program development. Provide clinical leadership and contribute to the advancement of patient care. Qualifications Registered Nurse with an Extended Class Certificate (Nurse Practitioner) from the College of Nurses of Ontario. 3–5 years of recent, relevant clinical experience preferred. Current BCLS certification. Knowledge of and ability to practice within the Ontario College of Nurses Standards of Practice for Nurse Practitioners. Valid Ontario driver's licence. Valid vehicle insurance with a minimum of $2 million liability coverage. PIECES and U-First training required. Preferred Skills Master of Nursing degree. Gerontological Nursing Certificate (GNC(C)). Experience providing care to older adults or within community-based healthcare settings. Requirements
Must be a Registered Nurse with an Extended Class Certificate (Nurse Practitioner) from the College of Nurses of Ontario and hold a valid driver's licence. 3-5 years of relevant clinical experience and BCLS certification are preferred.
