Support the client through care transitions by collaborating and connecting the client with community support services in their local community. Requirements Current Certificate of Competence with the College of Nurses of Ontario. Availability to work days, evenings, nights and every other weekend. Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire. Perform comprehensive client assessments and develop evidence-based care plans tailored to unique client needs. Act as the point of care lead for assigned caseloads and collaborate with community support services to facilitate care transitions.
