The Sherwood Park – Strathcona County Primary Care Network works as a team with family doctors and other healthcare professionals to advance the health and well-being of the communities we serve. We are one of the 40 Primary Care Networks (PCN) in Alberta. PCNs strive to improve integration of patient care, disease prevention and raising the general health of Albertans, chronic disease management, capacity and patient access, and the overall health and wellness of patients.

POSITION SUMMARY Reporting to the Clinical Manager, or designate, the Behavioural Health Consultant (BHC) supports the delivery of Primary Care Services in the Family Physicians office and/or PCN central office setting. The BHC uses clinical processes as a member of, and in collaboration with, the interdisciplinary team, in the delivery of evidence based high quality primary health care services that meet the needs of the patient.
