Key Responsibilities:

• Conduct psychosocial assessments and provide follow-up (including phone support) for individuals experiencing vulnerability or housing instability.
• Explain intervention objectives within a structured psychosocial support framework.
• Facilitate awareness and support activities promoting well-being, social inclusion, and personal development.
• Collaborate with internal teams and partners within the community network and supportive housing services.
• Monitor and follow up on individual and group interventions.
• Develop and update individualized intervention plans.
• Perform other related duties to support daily operations of the organization and housing services.

Requirements:

• Availability to work alternating day/evening shifts and 1 weekend out of 2.
• College diploma or bachelor’s degree in a related field (Social Work, Psychology, Criminology, Special Education).
• Experience in social reintegration, homelessness, and mental health.
• Strong knowledge of community and government resources.
• Proficiency with Microsoft Office Suite.
• Valid certifications: CPR/First Aid and overdose prevention (Naloxone).

Salary: $24 to $26 per hour, based on experience Schedule: Day shifts + on-call rotation (1 week out of 2) with compensation Status: Permanent
