Lieu CLSC de Sainte-Anne-des-Monts – CISSS de la Gaspésie Période d'affectation Du 26 juillet au 18 septembre 2026 Horaire Temps complet (35 h/semaine) Quart de jour Exigences Membre en règle de son ordre professionnel Minimum 2 ans d'expérience comme travailleur(euse) social(e) Baccalauréat en travail social Permis de conduire valide Formation OCCI Formation en évaluation psychosociale dans un contexte de régime de protection CV à jour Voiture requise Principales responsabilités Évaluer les besoins des usagers à domicile et mettre en place les services nécessaires au maintien à domicile Offrir un soutien psychosocial aux usagers Accompagner et soutenir les proches aidants Collaborer avec les différents intervenants afin d'assurer la continuité des services Intéressé(e) ?
