Through the provision of patient and non-patient care activities, the PT enhances the quality of life of patients by assessing, planning and implementing individually designed programs of physical treatment to maintain, improve or restore physical functioning and cardio-respiratory health, alleviate pain and prevent physical dysfunction in patients. Principal responsibilities include: Duties Providing assessment, intervention, consultation and education services for patients in the Stroke Outpatient Program. Collaborating with inter-professional team in providing patient focused care. Communicating effectively with patients, families, colleagues and external stakeholders. Developing and implementing creative comprehensive patient-centred care plans based on evidence and Stroke Practice standards. Consulting with internal and external stakeholders regularly to facilitate smooth and timely patient transitions. Facilitating and leading patient groups, as appropriate. Participating actively in the planning, implementation and evaluation of the program. Participating in professional and organizational initiatives supporting best practice. Quality management and research; providing education to students, as appropriate. Provide cross-program coverage as needed. Qualifications Completion of Masters Degree or equivalent in Physical Therapy Current registration with the College of Physiotherapists of Ontario Membership in CPA preferred Basic Cardiac Life Support (C Level) Heart Saver Plus Canada (on employment) Minimum 2 years clinical experience in neuro rehabilitation required; with the Stroke population preferred Excellent interpersonal and communication skills (verbal & written) required Strong customer service focus and excellent problem-solving skills required NDT training is an asset Additional Information Why join UHN? The physiotherapist will assess, plan, and implement physical treatment programs for patients in the Stroke Outpatient Program to restore physical functioning and alleviate pain. They will collaborate with an inter-professional team to provide patient-centered care and facilitate smooth patient transitions.

Candidates must hold a Master's degree in Physical Therapy and be registered with the College of Physiotherapists of Ontario. A minimum of 2 years of clinical experience in neuro-rehabilitation is required, with preference given to those with stroke population experience.
