Based in Mulgrave, this role supports clients across metropolitan Melbourne, including the northern, inner south-eastern and outer eastern suburbs, requiring regular travel to build meaningful relationships and coordinate personalised services that enhance wellbeing, independence and quality of life. In This Role, You Will Coordinate person-centred services within the Support at Home environment, supporting clients to achieve their goals and remain independent at home Deliver high-quality, person-centred care and support to clients in their homes and communities Conduct comprehensive assessments, develop individualised care plans and coordinate services Empower clients and carers to make informed choices about their care and support Manage your own client portfolio, including budgets, services and changing care needs Ensure services are delivered in line with aged care quality standards and organisational policies Maintain accurate case notes, documentation and client confidentiality Work collaboratively within a supportive, multidisciplinary team and escalate concerns where required Work towards agreed KPIs and service delivery targets, balancing quality client outcomes with productivity and service growth. Requirements
Candidates must hold at least a Diploma qualification in a relevant field and possess experience in case management or care coordination within the aged care sector. A current Victorian Driver's Licence and the ability to pass a National Police Check are also required.
