Yakima Valley Farm Workers Clinic is one of the largest community health centers in the Pacific Northwest. Our mission: "Together we transform our communities through compassionate, individualized care, eliminating barriers to health and well-being." We are dedicated to providing our patients with the high quality, culturally sensitive, affordable health care. We provide comprehensive medical, dental and social services in communities across Washington and Oregon. Our medical and dental services are complemented by behavioral health services, nutrition services, drug and alcohol treatment, an HIV/AIDS clinic, community health services, the Northwest Community Action Center (providing employment, training, mentoring, case management, and referral services), and mobile health units. We employ over 2,000 regular staff and another 338 residents, administrative fellows, students, and volunteers. Yakima Valley Farm Workers Clinic is a 501(c)(3) nonprofit organization, recognized by HRSA, The Joint Commission, and The National Committee for Quality Assurance for our accomplishments in community health care. The Care Coordinator manages the Wraparound with Intensive Services (WISe) program by facilitating Child and Family Team meetings and developing individualized care plans. They provide 24-hour crisis support and case management to stabilize high-risk youth and their families within the community.
