As an LPN in our Assisted Living Community at Hallmark on the Park, you will lead the community team, administer medications, provide wound care, and assess residents' needs. You will play a key role in maintaining continuity of care for our Assisted Living residents About The Role As an LPN at Baptist Housing’s Hallmark on the Park Community, you’ll apply the Wellness Model of Care, supporting resident independence while ensuring their safety and well-being. You’ll work closely with the Wellness Manager for complex cases and support the implementation of wellness programs under their guidance. Casual openings are for Days and evenings/week. No nights. Key Responsibilities Resident Care & Clinical Support Deliver nursing care in accordance with BCLNP Standards of Practice and Code of Ethics. Administer medications, provide treatments, and conduct ongoing assessments. Assist in developing, implementing, and evaluating care plans to support resident wellness. Consult with the Wellness Manager on complex medical needs and interventions. Leadership & Team Collaboration Support and mentor team members through training and supervision. Foster strong relationships with residents, families, and healthcare partners. Lead by example, demonstrating accountability, professionalism, and teamwork. Assist with scheduling and replacement calls when needed. Documentation & Communication Maintain accurate resident records and update care plans as needed. Enter resident information into Point Click Care / Seniors Care. Communicate any changes in resident health or incidents to the Wellness Manager Provide thorough shift reports to ensure continuity of care. Professional Growth & Compliance Maintain active LPN licensure and Standard First Aid certification. Engage in ongoing education, training, and professional development. Adhere to Ministry of Health and Health Authority regulations and best practices. Health & Safety Promote a culture of health, safety, and emergency preparedness. Identify and implement workplace safety improvements. Respond effectively to emergencies and support a safe care environment. Requirements Licensed Practical Nurse Diploma from a recognized post-secondary institution Good standing and membership with the British Columbia College of Nurses & Midwives (BCCNM) Current First Aid Certificate Current Food Safe Certificate About You You are a compassionate and empathetic LPN who connects with residents on a personal level, going beyond your duties to exceed their needs. Requirements
Candidates must hold an LPN Diploma and be in good standing with the British Columbia College of Nurses & Midwives (BCCNM). Current First Aid and Food Safe certifications are also required.
