About The Role As an LPN, you will play a critical role in supporting the physical, emotional, and social well-being of our residents. You will provide hands-on nursing care, including medication administration, assessments, and the development and implementation of individualized care plans. Working closely with Registered Nurses and the interdisciplinary team, you will support complex care needs while ensuring the highest standards of safety and quality. You will also contribute to wellness initiatives that enhance residents’ quality of life. By building strong relationships and maintaining accurate documentation, you help foster a culture of compassionate, resident-centered care. Responsibilities: Provide direct nursing care, including assessments, treatments, and medication administration Collaborate with Registered Nurses (RNs) and the care team to deliver comprehensive care Develop, implement, and update individualized care plans Monitor resident health status and promptly report changes to the appropriate team members Maintain accurate and timely documentation using electronic health systems Promote health, safety, and infection control practices Support resident engagement and overall well-being Requirements Graduation from a recognized Licensed Practical Nursing Program Current registration and good standing with BCCNM Experience in long-term care or assisted living preferred Strong clinical knowledge including medication administration and resident assessment Ability to work collaboratively in a team-based environment Excellent communication and organizational skills Compensation And Benefits An hourly wage of $32.84 – $41.35.
