END DATE MAY 2027) REASON TO POST: DIRECT REPLACEMENT WAGE RATE: $47.05- $60.98 Reporting to the Manager of Interprofessional Practice, the Social Worker collaborates with the interprofessional team to develop a plan of care. The Social Worker conducts a bio-psycho-social assessment, applies social work theory and methods and develops care plans with the goal of maximizing social and emotional functioning, preventing problems and situations that reduce coping capacities and independence. The Social Worker addresses barriers to optimal recovery or adjustment and discharge from hospital as well as support in end of life situations. Work Assignments may vary but will focus on the Surgery, Oncology, Ambulatory and Emergency departments. Required Qualifications / Skills / Abilities: Successful completion of a Master of Social Work Degree Current registration in good standing with the College of Social Workers Two to five years of recent related experience in a medical clinical setting Excellent professional judgment, conflict resolution and interpersonal communication skills with patients, families, team members, students and community partners Competence in the practice of evidence-based counselling and supportive therapy and inter-professional care, with experience in the application of practice models and outcome measures Experience with geriatric patients, end of life support and MAID situations Working knowledge of Mental Health legislation and the Trillium Gift of Life process Effective time management and organizational skills Excellent written and verbal communication skills Reliable, flexible, creative, self-directed professional able to work effectively and collaboratively with the interdisciplinary team as well as families and community partners Availability to work all shifts, including weekends and available for multi-site coverage when required Recent satisfactory performance and attendance records Demonstrate, promote and encourage Brightshores’ values – We Show Up for Each Other, We Are Trusted to Act, We Create Solutions Together, We Strive for Better, We Share Accountability Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at Brightshores through patient safety knowledge, skills and attitudes Comply with all applicable health and safety policies, procedures and regulations, and actively contribute to maintaining a safe and healthy work environment

Requirements

Candidates should have strong clinical judgment, counselling and communication skills, experience with geriatric and end-of-life care, and the ability to work flexibly and collaboratively across shifts and sites.
