THE ROLE The Family Transition Advisor (FTA) is based at the Transition Centre in Halifax and provides regional Military to Civilian Transition services to families of Canadian Armed Forces members across Nova Scotia and Newfoundland. As the family’s primary point of contact for transition support, the FTA helps families understand the transition process, identify strengths, priorities and potential challenges, and develop a personalized Family Transition Plan. Using a confidential, family-centred and strengths-based approach, the FTA completes intake and needs assessment, supports transition planning and navigation, provides information and education, and coordinates referrals to Military Family Resource Centres, community services and other partners. The FTA leads family navigation and transition planning before release and works with the family’s consent to support coordinated service delivery. The FTA collaborates with Transition Centre personnel, Family Liaison Officers, Veteran Family Program Coordinators, Military Family Resource Centres and other service partners to provide consistent and seamless family experience. The role maintains current knowledge of resources across the area of responsibility, develops regional and community partnerships, completes warm handoffs when ongoing or specialized support is required, and contributes service data and family feedback to strengthen national consistency, program quality and continuous improvement. This client-focused role requires strong communication, collaboration, and relationship-building skills, as well as adaptability, sound judgment, and leadership. The successful candidate will be responsible for non-clinical case coordination and must be able to work independently within a national service delivery framework. Qualifications Needed Education, Certifications and Licenses College diploma or certificate in Social Service, Adult Education, Community Development or a related field AND some years of experience in human sciences or a related field Experience In developing and delivering information sessions to groups and individuals In working with adults, children, and youths In community development In working with a wide cross-section of the community representing diverse backgrounds, cultures and social-economic circumstances and divergent goals In building community partnerships In mental health industry, an asset In crisis intervention, an asset In group facilitation In maintaining databases and/or spreadsheets In creating visual aids and support material In conducting client needs assessments and coaching In organizing, implementing and evaluating programs Competencies, Skills And Abilities Client focus, organizational knowledge, communication, innovation, teamwork and leadership. This Halifax based position provides support across Nova Scotia and Newfoundland and requires virtual service delivery, regional travel and scheduling flexibility. The successful candidate must be able to work effectively within a secure, privacy-conscious environment and maintain a valid driver’s license. Interviews will be conducted virtually. As part of the selection process, the successful candidate must successfully complete a Reliability status security screening. Requirements
A college diploma or certificate in social service, adult education, community development, or a related field is required, along with some experience in human sciences or a related field. Candidates need experience in areas such as needs assessment, coaching, group facilitation, community partnerships, program delivery, and working with diverse populations; a valid driver’s license and successful security screening and Vulnerable Sector Check are also required.
