CBI Home Health is looking to hire an Integrated Care Lead in Hamilton. Salary Range: The salary range for this position is $75,000 – $90,000 per year. What you will do As an Integrated Care Lead in CBI Home Health’s specialized, integrated care programs, you will play a pivotal role in shaping seamless, patient-centered transitions from hospital to home, ensuring every individual receives the right care at the right time. Partnering closely with hospital teams, community partners, and families, you’ll create comprehensive care plans that not only support safe discharges but also empower patients to thrive sustainably at home. Requirements
Candidates must be a Registered Nurse in good standing with the CNO and possess 3-5 years of supervisory experience. Experience in both home/community care and acute care settings is required, along with a valid driver's license.
