CBI Home Health is looking to hire an Integrated Care Lead in Hamilton. Salary Range: The salary range for this position is $75,000 – $90,000 per year. Partnering closely with hospital teams, community partners, and families, you’ll create comprehensive care plans that not only support safe discharges but also empower patients to thrive sustainably at home. Requirements

Candidates must be a Registered Nurse in good standing with the CNO and possess 3-5 years of supervisory experience. Experience in both home/community care and acute care settings is required, along with a valid driver's license.
