As an OT in our Integrated Care Program, you’ll support safe transitions from hospital to home through functional assessments, personalized interventions, and collaboration with care teams — making a real impact where it counts. What you will need to be successful * Professional Standing: You are a registered member in good standing with the College of Occupational Therapists of Ontario. * Mobility: Ability to travel within the community - valid driver’s license and insurance required * Client Focused: You embrace person- and family-centered care with passion and empathy. * Critical Thinking: You exercise good judgment and critical thinking in client care decisions. * Collaborative & Independent: Known for your excellent communication skills, you thrive in both team settings and working independently. * Home Care Advocacy: You value the role of a therapist in enabling clients to remain at home when it aligns with their personal goals.
