The physiotherapist will provide hands-on, community-based care to help patients rebuild strength and function after hospitalization. They will work within an interdisciplinary team to reduce readmissions and support patients in living safely and independently at home.

Candidates must be registered in good standing with the College of Physiotherapists of Ontario. A valid driver’s license, personal vehicle, and car insurance are required for travel between patient homes.
