Job summary
Care Coordinator – RPN Belle River Retirement Residence, Part Time, Temporary, (3:00PM-11:00PM) Week 1: Tuesday, Sunday. Week 2: Sunday-Wednesday. The Opportunity We’re looking for a Care Coordinator (RPN) to act as a supervisor providing the highest standard of care services. This role will assist in maintaining a safe and secure environment for residents, their families’ and other staff members while mentoring and providing direction and education to our Resident Guest Attendants for care and other team members’. What You’ll Do * Effectively utilizing the nursing process to provide high-quality nursing care to residents * Supervisory duties including ensuring excellent care services to Seasons’ residents and providing mentoring/education/guidance to care team on an ongoing basis * Assessments of residents moving into Seasons community and following up with residents throughout the transition to community (6-month re-assessments of residents per RHRA regulations, as health care changes) * Establish priorities based on individual resident needs * Providing person centered service plans * Ensuring compliance with Resident Assessments/Service Plans (PCC) while following community specific policies, procedures, and all applicable regulations * Risk management by adhering to thorough monthly care related audits * Medication administration (PCC knowledge an asset) What You’ll Bring * Experience working with seniors in a retirement or long-term care facility environment is considered an asset * Current Registered Practical Nurse License and is in good standing with the CNO * Point-Click-Care (PCC) knowledge is considered an asset * Strong verbal and written communication skills * Must be prepared to respond to emergency situations * Strong leadership and supervisory skills in relation to both residents and care staff * Current First Aid and CPR certification What We Offer * A safe, inclusive workplace where you’re supported and appreciated * Competitive compensation and benefits * Opportunities for leadership development and career growth * A warm, welcoming culture inspired by our vision of making people’s lives better, driven by our mission to create happy, healthy, and meaningful lives for our residents Additional Requirements * Clear Criminal Background Check, VSS and TB Test where required At Chartwell, we’re all about Making People’s Lives BETTER: the lives of our residents and their families, and the lives of our employees.
