Custom Health connects in-home technology, pharmacy, and pharmacist-led clinical care to create continuous visibility into medication use and support timely action when it’s needed. Powered by our proprietary AdhereNet® platform, our model connects medication delivery, in-home medication-use and health signals, and pharmacist-led clinical care in one coordinated system. Information from medication use, connected vitals, patient-reported insights, and ongoing patient engagement flows into AdhereNet®, helping our clinicians understand what is happening, identify when support may be needed, and engage patients or coordinate with prescribing providers as appropriate. Our integrated pharmacy network across Canada and the United States is an essential part of that infrastructure, connecting medication fulfillment and delivery with technology, medication-use insights, and our pharmacist-led clinical team. Together, these capabilities allow Custom Health to support patients beyond the point of dispensing. Custom Health works with patients, prescribing clinicians, health plans, healthcare organizations, and other partners to support better medication management at home and help connect medication-use insights with timely care.
