This role includes a combination of outpatient clinic visits and dialysis rounding, providing comprehensive care to patients with chronic kidney disease and related conditions. Key Responsibilities Provide high-quality, evidence-based care to nephrology patients in both clinic settings. Conduct thorough patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians. Perform dialysis rounding, including evaluating patients, managing medications, and coordinating care with dialysis center staff. Document all patient encounters and care plans accurately and efficiently in epic. Communicate clearly with patients, families, clinic staff, and interdisciplinary teams. Support nephrology providers by assisting with patient follow-up, education, and chronic disease management. Participate in quality initiatives, care coordination, and process improvement efforts. Qualifications- Graduation from an approved school for Physician Assistant Or Nurse Practitioner-Masters Degree. Requirements
Candidates must be a licensed Physician Assistant or Nurse Practitioner with a Master's degree and the ability to obtain licensure in Illinois. Previous experience of 1-2 years in Nephrology or a similar setting is preferred.
