The Community Health Representative works with the Community Nurse to deliver health prevention, surveillance, and treatment programs. They act as a liaison between patients, families, and healthcare providers to ensure effective care coordination and health education. Candidates must have 2-3 years of experience in a similar role and a Certificate or Diploma in Health Sciences. A valid driver's license and current CPR certification are required, along with the ability to pass a criminal record and vulnerable sector check.
