The Community Health Nurse works independently and collaboratively within an interprofessional team to manage a client caseload, providing assessments, interventions, and care planning. They facilitate client transitions across the healthcare continuum and provide direct care to support patients in living safely at home. Candidates must hold current practicing registration as a Licensed Practical Nurse with the BCCNM and possess at least one year of recent clinical experience in acute or community settings. A valid BC driver's license and access to a personal vehicle are also required for this position.
What you’ll do
