The Community Health Nurse provides direct clinical care, assessments, and care planning for clients with complex chronic conditions in a community setting. They collaborate with interprofessional teams and primary care providers to facilitate transitions and support clients in living safely at home. Candidates must hold current practicing registration as a Registered Nurse with the BCCNM and possess at least one year of recent related clinical experience. A valid BC Driver's license and access to a personal vehicle are also required for the role.

Requirements
